What Is a TMT?

Finding heart problems that rest hides

Some heart problems only appear when the heart works harder. During a TMT, you walk on a treadmill that gradually gets faster and steeper while a continuous ECG records your heart's response.

Doctors order a TMT to evaluate chest pain, palpitations, unexplained breathlessness, to screen for coronary disease in high-risk people, and to check fitness before demanding jobs or surgery.

The test takes about 30 minutes including preparation and recovery. The test is conducted by a trained technologist under the supervision of a doctor, with a defibrillator, oxygen and emergency drugs in the room. Our staff are trained in resuscitation.

Serious complications are rare (of the order of 1 in 10,000 tests), but because they are possible we explain the test and take your written consent before starting.

Before the test

Wear comfortable clothes and walking shoes. Avoid a heavy meal 2–3 hours before; no tea, coffee or smoking for a few hours before. Carry your medicines list.

During the test

Electrodes are attached, and you walk in stages — speed and incline increase every 3 minutes. Tell the staff immediately if you feel chest pain, dizziness or unusual breathlessness.

Afterwards

You cool down walking slowly for a few minutes; your heart rate and BP are watched until they settle. The report goes to your doctor the same day.

We will not start the test if you have had a heart attack within 2 days, unstable chest pain, an uncontrolled irregular heartbeat, severe breathlessness at rest, severe aortic valve narrowing, or other serious heart conditions — this is for your protection.

What is a TMT?
TMT (treadmill test / stress ECG) records your heart's response while you walk on a treadmill at increasing speed and incline. It helps evaluate chest pain, fitness and suspected coronary disease.
How do I prepare for a TMT?
Nothing to eat for 3–4 hours before. No tea, coffee, cola or energy drinks for 12 hours. No smoking for 3 hours. Bring your medicines list — some heart medicines (beta blockers) reduce the accuracy of the test, and only your doctor can tell you whether to skip a dose. Never stop a heart medicine on your own. We will not start the test if your blood pressure or ECG on arrival is unsafe.
Is a TMT safe? What are the risks?
